Teacher/Coach Licence Application Forms
Forms required for an initial or renewal of license
To apply for a teacher/coach license we require four main documents:
- Initial application form
- Evidence of your ASA qualification- if not completed through the ASA you would have needed to apply for exemption to attend an ASA qualification or apply for RPL
- CRB (if done through the ASA must have been issued within the last 3 years, if done through another organisation we need to see the original document along with a CRB consent form, this must have been issued within the last 6 months). If you need to gain a CRB please contact CRB@swimming.org
- Child Protection Certificate (if done through the ASA or Sports Coach UK a copy of the certificate can be sent in, if done through your local authority we must see the original certificate. All certifictaes must have been issued within the last 3 years). Please click here to see what child protection certificates the ASA accept. If you need to gain the Child Protection certificate please click here to search for a seminar in your region
Your license will be sent to you by post in the form of an official ASA teacher/coach license card which will be valid for 3 years from the date of issue. However, we must receive a completed annual return proforma, signed by your line manager/head coach and returned to us annually. This proforma explains the yearly expectations of being licensed.
